Description:
HCCH is a 501 (c)(3) non-profit organization founded by Rick Baxley, MD and a group of volunteer doctors concerned about the growing problem of illness and injury among the homeless. The doctors began operating as part of a local shelter but soon outgrew the site. In 1994, assisted by a generous start-up grant from the nationally acclaimed Robert Wood Johnson Foundation, HCCH was incorporated and developed a clinic facility to serve the ever-growing clientele that had no insurance or other source of medical care except hospital emergency rooms. The Orange Blossom Family Health Center was opened by HCCH in January of 2006 to further our mission to offer a systemic solution to move people out of homelessness and into self-sufficiency as well as offering a safety net to the financially precarious keeping them from falling into homelessness due to the expenses of a catastrophic illness or loss of income due to illness. The services at HCCH are provided in a multi-disciplinary approach combining aggressive street outreach with integrated systems of primary health care, dental, vision, behavioral health, education and patient advocacy.
