The mission of Community Care for the Elderly is to delay or prevent inappropriate or unnecessary nursing home admission of severely functionally impaired seniors.
Description:
Community Care for the Elderly (CCE) helps older adults maintain their independence and self esteem by providing services that enable them to remain living at home as long as possible. A CCE qualified care manager identifies frail, low-income individuals most at risk of nursing home admission, assesses their needs and develops a personalized care plan. Services provided, or arranged for, may include home-delivered meals, baths, personal hygiene supplies, home safety modifications, light house cleaning, yard clean-up, and ongoing care management.
